Enrollment Forms: Required Immunizations

To participate in skills labs and externship, all students accepted into the program must submit documentation for the required immunizations listed below.

You can begin uploading your immunization records through your Admissions Portal as soon as you’ve submitted your application.


Immunizations 

Covid-19
Per current CDC guidelines, NIMAA requires proof of documentation for at least one dose of the COVID vaccine.
Seasonal Flu
Students must submit documentation for the seasonal flu shot from the year you are participating.
  1. Students enrolling with a Spring start are required to submit their flu shot documentation during enrollment. For example, if you are applying for Spring 2026, your will submit your flu shot for the 2025-2026 flu season with the rest of your immunizations during the enrollment process. 
  2. Students enrolling with a Fall start are required to submit their flu shot documentation no later than December 1. For example, if you are applying for Fall 2025, you will submit your flu shot for the 2025-2026 flu season during the program, no later than December 1, 2025. Review this page for the steps to upload your flu documentation if you are a fall student. 
Tuberculosis (TB)
Students must submit a recent TB test that was completed within 6 months of the program start.

For example, if you are applying for the Fall 2025 cohort that begins on September 2nd, 2025, your TB test must be completed on or after March 2nd, 2025.

Measles (MMR)
If your current provider does not have a record of your MMR vaccine, you can have titer tests drawn to verify your immunity. 

If you do not have immunity to MMR, we recommend reaching out to your provider as soon as possible to schedule the immunization. MMR is a two-dose series that takes a minimum of 4 weeks to complete and this could delay or prevent your enrollment in the program. 
Varicella
If your current provider does not have a record of your Varicella vaccine, you can have titer tests drawn to verify your immunity. 
Hepatitis B
If your current provider does not have a record of your Varicella vaccine, you can have titer tests drawn to verify your immunity. 

If you never received the vaccine for Hepatitis B, you will need to receive the vaccine series. There is a two-step series and a three-step series. We strongly encourage you to ask your provider about the two-step series as the three-step series takes a minimum of 6 months to complete and this could delay or prevent your enrollment in the program. 
Td/Tdap
If your current provider does not have a record of your Td/Tdap vaccine, you can have titer tests drawn to verify your immunity. 

How to Upload Your Immunizations

  1. Download the required forms. Access the “Immunization Records” form in your Admissions Portal. You can also download the forms at the bottom of this article.
  2. Upload your completed documents. Submit your immunization records using the Immunization Records form in your Admissions Portal.

If accepted, you will receive access to your Populi account, where you can find and manage all of your forms. 

Frequently Asked Questions

Do I need to make an appointment with my provider to complete the forms?

Not always. Most clinics allow you to drop off forms to be completed without an appointment. However, if you need any immunizations, you will likely need to schedule one. 

Can I submit records from my patient portal instead?

Yes! You can submit a copy of your official immunization records (from your patient portal, for example) along with NIMAA’s immunization worksheet. If you do this, the worksheet does not need a provider signature.

However, if you submit the worksheet without supporting documentation, it must be signed by a provider or clinic staff. 

What if I don't have health insurance?

You have a few options:
  1. Contact your local health department. You can find yours using the NACCHO Local Health Department Directory.
  2. Visit the CDC’s vaccine locator page to find low-cost options.
  3. In some cases, your externship organization may assist with immunizations. If you’re unable to access the resources above, contact externship@nimaa.edu for support.

How do I request a medical or religious exemption?

To request an exemption:

  1. Go to your Immunization Records form in the Admissions Portal.

  2. Select "Yes" when asked if you're requesting an exemption.

  3. Choose medical or religious, then download and complete the exemption form.

Note: Exemption requests are reviewed by NIMAA’s Clinical Director and may be denied. Your externship site may have its own exemption policy, too. You can read more about exemptions in the NIMAA Catalog.


What if my provider doesn't have my immunization records? 

If your current provider does not have a record of your immunizations, you can have a titer test done to verify your immunity. If the test shows that you're not immune, you’ll need to receive the vaccine before participating in the program.

Does a Shingles vaccine count as Varicella?

No. Shingles and Varicella (chicknpox) are separate vaccines and not interchangable. 

If I got the flu shot in late winter, do I need to get one in the fall? 

Yes. The flu shot changes each year, so you’ll need to get the current season’s vaccine in the fall.
